Job Summary: Delivers home health care to VNA patients of all ages and families whose needs range from basic to complex. Provides skilled nursing care, including education in the patient's home environment.
Primary nurse to 25 - 30 patients:
Assesses patient needs and develops individual care plans, in collaboration with physician and patient. Completes a comprehensive clinical and environmental assessment of the patient. Revises plan according to patient need/physician orders. Responds appropriately to emergency patients needs. Monitors patient response to treatment and communicates appropriate information to patients, peers, managers, physicians and other providers. Participates in patient care conferences.
Qualifications:
Education: Graduate of an accredited school of nursing
Experience: 2yrs acute care staff nursing preferred. Home care experience, wound care, IV skills and good teaching skills, all a plus.
Licensure/Certification: Current licensure in State of New Hampshire as a professional nurse.
Driver's License: Required to travel throughout our care area to provide home health care services and other job-related activities.
