Join The Program –
Program participants may be referred by case managers, family members, or friends. All admissions are approved by a physician. Depending on the person, attendance in the program ranges from two to five days a week. Adult Day Services is a long-term program for most people. If it becomes necessary for a person to transition to a different setting, we will work closely with care providers and family members to make the change a success. The center will be open Monday through Friday 8am to 5pm. To make a referral or inquire, please call 603-332-1133 and ask to speak with our Program Mangager.
Caring, Skilled Staff –
Rochester District VNA Adult Day Center staff is skilled, is skilled, caring, and dedicated to improving people’s lives trough personalized care and attention. We work cooperatively with you and your doctor, family members and caregivers, to meet your specific needs.
A team approach including; nursing, therapies, social worker, licensed nursing assistant, program manager and volunteers will be used to best meet your needs.
Adult Day Center Specializes in –
- We consider the whole person: physical, mental, social, and spiritual needs.
- Creating a learning and sharing environment through interaction and engagement with staff and other clients.
- We encourage people to remain as independent as possible.
- Helping people remain in the community.
- Providing respite care for families.
Nursing Services –
- Blood Pressure monitoring
- Ongoing assessments and supervision
- If needed, individualized care plans would include skilled services per orders determined by your physician.

Recreational Activities –

Meals –
Each day we provide morning and afternoon snacks along with a nutritional lunch
