This program uses a combination of home visits, phone contact and telehealth. Case management emphasizes patient education by increasing adherence to medical and diet regimens, and recognizing early symptoms of exacerbation that could lead to re-hospitalization. Collaborative efforts between providers are essential to a successful disease management program. Our focus on teamwork and timely communication enables us to provide our patients with the best possible care.
