HOMECARE

Skilled Nursing - A licensed nurse visits a client in home to care for and monitor the medical condition and assist with special health care needs.

Licensed Nursing Assistants -A certified home health aide assists clients with personal care, home exercises and activities of daily living.

SPECIALTY SERVICES

Certified Wound/Ostomy Nurses - The goal of Rochester District VNA's Ostomy Management Program is to help the patient and /or caregiver to understand and manage ostomy care. Each ostomy patient will have an individualized plan of care designed to meet specific goals and outcomes.

Ostomy Management and Education includes:

Comprehensive stoma and skin assessment
Product selection
Pre and post op teaching
Stoma site selection and marking
Treatment of skin complications
Nutritional assessment and counseling

The ostomy management program follows the guidelines by Wound, Ostomy, Continence Nurse Society and the United Ostomy Association. A certified ostomy nurse on staff serves as a resource to staff and patients.

Wound Management & Education Program

The goals of Rochester District VNA's Wound Management Program is to help the patient and/or caregiver to understand and manage acute and chronic wound care; the patient will have an individualized plan of care designed to meet specific goals and outcomes.

Wound Management and Education includes:

Risk assessment
Prevention strategies
Product selection
Nutritional assessment
Multidisciplinary care coordination
Recommendations for treatment options.

The Wound Management Program follows guidelines by Agency for Health Care Policy and Research, and Wound, Ostomy, Continence Nurse Society. Certified wound care nurses on staff serve as a resource to staff and patients.

Certified Psych Nurses -The goal of Rochester District VNA's Psychiatric Nurse program is to provide special assessment and teaching for patients with mental/emotional disorders. Patients may be referred by physicians for psychiatric nursing evaluation in the home. Within the agency, any discipline may request a psychiatric nursing evaluation for a patient with a behavior problem or mental status changes. Each psychiatric patient will have an individualized plan of care designed to meet specific goals and outcomes.

The program includes:

Comprehensive mental health assessment
Multidisciplinary approach to care
Medical evaluation and recommendations
Home safety evaluation and management
Functional assessment
Integration of medical conditions

Rochester District VNA Psych Nurses are Medicare approved and certified by the American Nurse Credentialing Center in Psychiatric and Mental Health Nursing. Certified psych nurses on staff serve as a resource to staff and patients.

Certified Lactation Consultant - The goal of a lactation consultant is to educate, guide, and support the patient with the many questions and available options concerning breastfeeding, skin/nipple care, nutrition, and equipment. A lactation consultant is a valuable resource for both fellow staff members and patients in our efforts to provide the best care and information to aid patients with their health care choices, and is an integral part of our Maternal/Child Health services.

Diabetic Educator - The goal of Rochester District VNA's Diabetes Program is to help the patient and/or caregiver better manage and understand diabetes. Persons newly diagnosed with diabetes or experiencing a change in treatment or long-term complications will benefit from the program. Each person with diabetes has an individualized plan of care designed to meet patient specific goals and outcomes.

The program includes education in:

The diabetes disease process
Diabetes medication/insulin education
Nutrition
Blood glucose monitoring
Hypoglycemia/hyperglycemia recognition and treatment
Insulin Therapy
Foot care, education and assessment
Sick day management
Long-term complications of diabetes
How to live with a chronic disease

Our multidisciplinary team approach to diabetes management promotes better diabetes control and achievement of a healthier life style.

Your VNA's diabetes management program follows the American Diabetes Association Standards of Care. A diabetic educator on staff serves as a resource to staff and patients.

Certified Intravenous Nurse - The goal of Rochester District VNA's Infusion Therapy Program is to help the patients and/or caregiver to independently understand appropriate IV management with education and support. It is very common to receive different treatments in the home, some patients may become independent with their care in a short time while others may need ongoing support and care.

Infusion therapy treatment services includes:

Ongoing education and support
Signs and Symptoms of Complications
Management and education of computerized pumps to regulate infusions
Disease Management
Pain Management
Antibiotic treatment
Immunoglobulin therapy
Chemotherapy
Hydration
Total Parenteral Nutrition (TPN) and Enteral Nutrition (G-tube, PEG)
Maintenance of various lines including:
- peripheral lines
- PICC lines
- Midlines
- Port-o-caths
- Central lines, multiple types

Newborn-Post Partum/Pediatric Nurses -Our Maternal-Child Health Nurse can provide the educational support and care new mothers and infants need in their first few weeks at home. There is also a lactation consultant on our staff that will assist you with any breast-feeding help you may need. A visit from our maternal-child health nurse can be arranged by your physician or hospital to ease your transition home. We can set up a time when we can visit you and your new baby in your home for an exam, teaching or just answering your questions. It doesn't matter whether this is your first child or an addition to your growing family. Rochester District VNA is ready and willing to help you.
